ID Cards
If you are an enrollee with Delta Dental Premier® or Delta Dental PPOSM coverage, you don't need an ID card. When visiting a Delta Dental Premier or Delta Dental PPO dentist, simply provide your social security or identification number. The dental office can use that information to verify your eligibility and benefits.
Print an ID Card Via this Site
If you still would like an ID card, you can print a customized ID card on demand. Just follow our easy, three-step registration process and log in to our online benefits and eligibility system. Once you've logged in, click the "Eligibility & Benefits" tab to view your eligibility and benefits information and to print an ID card.
