How to file a dental claim

Delta Dental dentists agree to handle all claims and paperwork for you. But, if you visit a non-Delta Dental dentist, you may need to file the claim yourself.

If you do need to file a claim form:

1. Download the claim form for the Delta Dental organization administering your benefits.

• Delta Dental of Delaware
• Delta Dental of the District of Columbia
• Delta Dental Insurance Company (Alabama, Florida, Georgia, Louisiana, Mississippi, Montana, Nevada, Texas and Utah)
• Delta Dental of New York
• Delta Dental of Pennsylvania (Pennsylvania and Maryland)
• Delta Dental of West Virginia
• AARP Dental Insurance Plan web site
2. Complete lines 1 through 15 on the claim form.
3. Attach a copy of the dentist's Statement of Treatment, including the dentist's name, complete phone number, and a description of each service that the dentist performed.
4. Make a copy for your records.
5. Mail the original copies to the address printed on the form.

We usually process claims within two weeks unless additional information is required from you or the dentist.
